Social Servcies

Part of our commitment to the quality of care is to support you and your family through these trying times. Illness and hospitalization can cause disruptions in our physical health and in our ability to cope with difficult decisions. Social Workers are licensed professionals who can provide you and your family with emotional support through:

  • Counseling

  • Crisis intervention

  • Support groups

  • Community resource information
Social Services is also an integral part of the end-of-life healthcare team alongside the nurse and physician. They work closely with you and the medical team to begin the difficult discussion of palliative care; to identify the patient wishes regarding treatment or hospice; to identify the place of death, and to identify the Advanced Healthcare Directives. Social workers address the patient's and family's grieving and educates the family on bereavement services available to them in the months after the loss. They also educate the family on funeral or cremation options in the community.

You may contact your Resource Coordinator or Social Worker by asking your nurse to contact them or you may call the Social Services department at 949/764-8225.
